ESSENTIAL VII
Interprofessional Collaboration for Improving Patient
and Population Health Outcomes
Recognizes that the master’s-prepared nurse, as a member and leader of interprofessional teams, communicates, collaborates, and consults with
other health professionals to manage and coordinate care.

EXEMPLARS
I value myself as an effective team member, communicating effectively and consulting appropriately. I have room to grow in organizing consults and coordinating results from multiple providers, however, I know this will come with time.
This SOAP note provides a great example of collaboration. This patient is seen in the office for addiction treatment and is 8 weeks pregnant at the time of this SOAP note. The obstetrician she sees is one of few in the Knoxville area that will treat mothers who suffer from opioid dependency. She is one of several patients seen in the office for addiction treatment and it is in collaboration with this obstetrician that mothers can be successfully treated and even weaned from Subutex prior to delivery.
Our pediatric service learning project required us to work together with non-healthcare related entities (in this case, the boy scouts). Bullying is an area that can impact the overall health of both the pediatric and adult patient and we are required to work with other members of the healthcare team to promote general health.
Autism is a diagnosis that has seen a dramatic increase in awareness and age of diagnosis. This presentation discussed early screening tools as well as theoretical educational seminars and website usage to keep track of patients who have screens as concerning a potential developmental disorder and the rate of intervention. This essential requires me to work with other healthcare systems to track interventions as well as formal diagnosis and advocacy for early intervention and treatment.
This group project discusses pain management using a non-numeric pain scale. This project met this essential as we greatly discussed how we would collaborate and discuss pain management with other disciplines and give the patient the best options and alternatives to pain management.
